Hospitals and healthcare is currently the most active sector for P3 in Canada, with many projects in procurement or complete in British Columbia, Ontario, Quebec, and New Brunswick. They range in size from the 100,000-square-foot Paramedic Service Headquarters in Ottawa, Ontario to the 772-bed Montreal University Health Centre (CHUM) project in Quebec. The preferred model has been the design-build-finance-maintain (DBFM), including hard facilities management and some other non-healthcare services such as food service, portering and security. With few exceptions, such as the private Shouldice Hospital in Thornhill, Ontario, governments have chosen to retain the provision of healthcare as a public service.

Canada's first major foray into P3 schools began in Nova Scotia in 1997 when the government issued a Request for Proposals for 39 schools under a net lease model (6) and turn-key model (33). The program was cancelled in 1999 following a change in government and two of the three proponents subsequently contracted back the school operations to school boards.
